FOR IN OFFICE APPOINTMENTS:

• Before your appointment, please provide your insurance information and complete online check in forms below. 

• ​Please postpone your visit if you, or someone you've recently had contact with, have experienced cold or flu symptoms within the last 14 days. 

• As of April 3rd, 2023, Washington state ended the masking requirements for healthcare facilities.  While masks are no longer required, we still strongly recommend you wear a well-fitting face mask during your visit.  In addition, our staff members will continue using masks to promote staff and patient safety. 

• Please arrive 5 minutes before your scheduled time.

• We kindly ask that you come to your appointment by yourself.  However, pediatric patients, or patients requiring assistance, may bring one accompanying guest.  If you would like to have a guest accompany you while selecting frames, we simply ask they please wait to enter the office until you have completed your exam and are ready to select frames.  

• Please help us adhere to social distancing upon arrival.  If the front desk is occupied, please form a line, and wait to approach the desk until the previous patient has left.  Thank you for your patience!

• Please rest assured, all exam equipment will have been thoroughly cleaned and sanitized before you arrive.

• Please note we are no longer accepting cash/check payments but are glad to process payments by credit card or Apple Pay.
